Nissa was about 6mos old before I considered her to be housebroken. I considered this a long time. Riley came to us already housebroken so I don't know how long it took for his previous owner to train this.
She seems to have the concept down but she just doesn't seem to have the physical strength to hold it a little longer to make it outside in time. This is good
I don't remember for sure but I think I may have been getting up for middle of the night potty breaks a few weeks after we got Nissa. You *are* a new Mom all over again

Certainly do have her checked by the vet if you're concerned. It's probably about time for a vet check anyway, so tell them about this so they can look for a cause if need be. You need to know if there's a physical problem or not. You don't want to wind up working on housebreaking for a few months only to find out that she's got a health issue that prevents her from housebreaking. You'll only frustrate both of you and if it is physical it may be something painful.
When it comes to crate training, the crate should be just big enough for her to turn around it and lay down comfortably. You need to make adjustments if it's to big. The idea is that a pup will not wet/mess where it sleeps. But make sure to keep an eye that you don't let her get to big before you adjust the crate size as time passes.
Cut down a little on her water consumption a few hours before bed so she doesn't have so much to get rid of during the night.
